35M with diabetes and liver disease

Hi, I am Arefin Sadat, a final-year MBBS student. This is an E-Log that depicts the patient-centred approach to learning medicine. A 35 years old male presented with the complain of-


1)Burning micturition for 1 year and abnormal whitish fluid in urine for 3 days.

2)Generalised weakness of whole body that make him unable to move.

3)Diabetes


History of present illness:


They patient was reasonably well 8 months back. Then he developed chicken pox which persisted for 1.5 months. After 2 months of getting recovered from pox, he developed fever and vomiting.


The fever persisted for almost long 6 months, which was occasional in nature(sometimes no fever for 1/2 days, then again affected with fever), and subsided with medicine. The temperature tends to elevate after afternoon and persisi till mid-night. Patient states that, for the last 24 days, there is no fever.


On dec,2022, he got admitted into the hospital where his urine test and blood test was done and he was diagnosed with diabetes.


One fine morning, he took insulin and at night(2'o clock), he became senseless. When taken to hospital, following providing sugar, he got his sense.


Since dec,2022, he started feeling weak, but for the last 5 days, it just git so severe that, he even cant move by himself.


He also had severe vomiting for 2 days on 5 days back(7/8 times on 1st day, and 3/4 times on 2nd day). He also stated that, he developed  anorexia for last last 5 days.

Patients had constipation for last 10 days and no defecation at all for last 4 days.


The patient added that, he always sweat a lot and feel so warm from inside the body. Even in winter he doesn’t need warm cloths 


History of past illness:

Patient got bitten by dog at the age of 8 years and lated got vaccinated.

He is a known case of asthma.


History of family history:

His father is one-sided paralysed.
