70 years old male with Renal Stone

E-LOGS MEDICINE

Hi, I am Arefin Sadat, a final-year MBBS student. This is an E-Log that depicts the patient-centred approach to learning medicine. This E-Log has been created after taking consent from the patient and their relatives. The links that were used by me for understanding the available data on the particular disease have been mentioned below in each post. I hope you learn valuable information after giving it a good read.

CASE OF A 70-YEAR-OLD MALE WITH RENAL STONE

This is a case of a 70-year-old male who was a farmer in profession. This de-identified blog was created after taking informed consent from the patient.

CHIEF COMPLAINTS

  1. Kidney stone for more than 4 years.
  2. Mild and occasional back pain for 4 years.

History of Presenting Illness

He was reasonably well till 2017. Then gradually, he noticed that his urination was not complete. Even after urination, within 30/40 minutes, he again felt the need for urination.

He then consulted with a doctor, and at that time, he was diagnosed with a bladder stone in 2017.

Later in 2018(Nov), when the problem got severe, they again consulted with a gastroenterology specialist. At that time, he was diagnosed with both urinary stones and renal stones, along with a horse-shoe shape kidney.

After that, he went to Kamineni Institute of Medical Science at the end of 2018, where the doctor removed his bladder stone and suggested visiting 1 month later for the operation of renal stone.

After the removal of bladder stone, the patient was completely well, and they didn’t go for the renal stone operation at that time.

The patient also complains of mild, occasional back pain since 2017, only when he does some heavy work. Occasionally, he takes a painkiller for the backpain.

Currently (Nov 2022), the patient has been admitted to the hospital for a renal stone operation.

The patient is not a known case of DM, asthma and HTN.

He does not have any significant family history.
